The Contractor will use the ESS-C-157 Pre-Placement Form for the initial job placement only in the Supported Employment (SE), Employment and Retention (ER), Job Placement (JP), JOBS, and SES contracts (Onsite Support and Training Service and Extended Services for Transition Service). The form will be submitted by email to the DRS Counselor and ESS TA on or before the first day of employment. Retain in the case file and submit a copy with documentation for payment.
