ESS-C-430 Stabilization Milestone Report (Rev 2022-09-12) form will be used by the EC during the SE Stabilization milestone in the Supported Employment (SE) contract to document the individual's level of performance during the milestone. The EC will also document the strengths identified and how they are being reinforced, areas identified as needing improvement, supports and training provided, and ability to interact with supervisor and co-workers. The EC will document whether or not the team meeting was conducted.
