The ESS-C-425 SE R8 Milestone Report will be completed by the EC after completion of the SE R8 milestone in the Supported Employment (SE) contract. The EC will document the individual's performance, areas needing improvement and steps taken to address the issue(s), the training and supports provided by the EC to assist the individual in maintaining employment, and how well the individual interacts with co-workers and supervisor. Only weeks where the hours worked met or exceeded 75% of the weekly work goal are counted toward the minimum eight (8) weeks. The weekly work goal is stated in the Individualized Plan for Employment (IPE).
