The Natural Supports & Fading Plan has been combined with the Natural Supports Cooperative Agreement. The EC will complete this document with the individual and the employer. The EC will print the document and verify all parties understand and agree to the Natural Supports and Fading Plan. Both the individual and the employer will write their initials indicating their agreement with the plan. The Natural Supports & Fading Plan will be reviewed and updated when needed. The EC will only submit the Natural Supports and Fading Plan that has been updated and all parties have signed their agreement to the changes.
